Go to the DOH website to log into your account as a returning user.

Review your contact information in "Profile Settings"

Make sure to review all information on your 'Profile Settings' tab to make sure that the contact information is correct and that the address is still correct and where you currently reside. If you need to make changes, be sure to click the 'save changes' button at the bottom.

Make payment to the state

Click the ‘Make Payment’ tab in the blue ribbon at the top. Follow the prompts to make the $25 of $50 payment to the state.
After renewing your registration, you'll receive an email letting you know that your next step is to obtain a new patient certification.
When this is done, we will be able to re-certify you once you have completed the re-certification form .
